Метод адгезивно-шинирующих конструкций

Abstract

Введение. При отсутствии 1 зуба в эстетической зоне зубного ряда врач-стоматолог определяет показания к выбору оптимального метода лечения, одним из которых является изготовление адгезивного или адгезивно-шинирующего протеза. Цель. Разработать метод адгезивного протезирования, применяемый при утрате зуба после травмы зуба с использованием адгезивно-шинирующих конструкций. Материалы и методы. Выделена группа исследования из 222 пациентов со следующими диагнозами: хроническая травма зуба – 122 пациента, острая травма зуба – 100 пациентов (перелом корня – 78 пациентов, вывих зуба – 22 пациента), имевших показания к изготовлению адгезивно-шинирующих конструкций. Результаты. Метод адгезивного протезирования зубов после травмы заключается в изготовлении адгезивно-шинирующего протеза, состоящего из 2 частей – шинирующей конструкции, иммобилизирующей подвижные зубы, находящиеся рядом с дефектом зубного ряда, и адгезивного протеза, имитирующего удаленный зуб. Показаны разные подходы к временному протезированию после острой травмы и постоянному при хронической травме, а также изготовление протеза при отрыве и последующем удалении зуба от ранее изготовленной шины. Заключение. Разработан новый метод протезирования при лечении перелома корня зуба, вывиха зуба и периодонтита и удалении одного из зубов с использованием адгезивно-шинирующей конструкции с учетом клинической картины и свойств армирующего материала.

Introduction. When one tooth is missing in the aesthetic zone of the dentition, the dentist determines the indications for choosing the optimal method of treatment, one of which is the fabrication of an adhesive or adhesive-retaining prosthesis. Purpose. To develop a method of adhesive prosthetics used in tooth loss after tooth trauma with the use of adhesive-retaining structures. Materials and methods. A study group of 222 patients with the following diagnoses was selected: chronic tooth trauma – 122 patients, acute tooth trauma – 100 patients (root fracture – 78 patients; tooth dislocation – 22 patients), who had indications for the fabrication of adhesive splinting constructions. Results. The method of adhesive prosthetics of teeth after trauma consists in the fabrication of adhesive splinting prosthesis consisting of 2 parts – splinting structure immobilizing mobile teeth adjacent to the defect of the tooth row and adhesive prosthesis imitating the extracted tooth. Diff erent approaches to temporary prosthetics after acute and permanent prosthetics after chronic trauma are shown, as well as fabrication of a prosthesis in case of tooth detachment and subsequent tooth extraction from a previously fabricated splint. Conclusion. A method of prosthetics in the treatment of root fracture, tooth dislocation and periodontitis and extraction of one of the teeth with the use of an adhesive splinting construction taking into account the clinical picture and properties of the reinforcing material has been developed.
