Реакция «трансплантат против хозяина» с проявлениями в полости рта у детей и молодых взрослых – реципиентов аллогенных гемопоэтических стволовых клеток

Abstract

Введение. Изменения в полости рта у взрослых пациентов с реакцией «трансплантат против хозяина» (РТПХ) описывают в диапазоне от диффузной гиперемии и эрозий до гиперкератоза, частым признаком РТПХ считают мукоцеле на нёбе, щеках и губах. Цель. Определение частоты случаев хронической РТПХ (хрРТПХ) с проявлениями в полости рта у реципиентов аллогенной трансплантации гемопоэтических стволовых клеток (алло-ТГСК) (детей и молодых взрослых), леченных по протоколам, принятым в Республике Беларусь. Материалы и методы. Изучены истории болезни 161 пациента Республиканского научно-практического центра детской онкологии, гематологии и иммунологии, проходившего терапию методом алло-ТГСК в период с 11.01.2011 по 20.05.2021. Результаты. Определено, что частота хрРТПХ с проявлениями в полости рта среди всех изученных случаев составила 11,8% и 41% всех случаев РТПХ. Большая часть случаев хрРТПХ с поражением тканей полости рта (81%) зарегистрирована у пациентов клиники, проходивших ТГСК для лечения лейкоза (доля пациентов с таким диагнозом среди вошедших в анализ – 70%); частота РТПХ с оральными признаками у пациентов с ОЛЛ составила 11%, с ОМЛ – наивысшие 23%. Заключение. Изучение архивных историй болезни детей и молодых взрослых – реципиентов алло-ТГСК позволяет заключить, что хрРТПХ с проявлениями в полости рта является актуальной проблемой.

Introduction. Changes in the oral cavity in adult patients with graft-versus-host disease (GVHD) are described in the range from diff use hyperemia and erosions to hyperkeratosis. One of common sign of GVHD it is considered mucocele on hard and soft palate, cheeks and lips. Purpose. The purpose of the study was to determine the incidence of cases of chronic GVHD (cGVHD) with manifestations in the oral cavity in recipients (children and young adults) of allogenic hematopoietic stem cell transplantation (allo-HSCT) treated according to the protocols adopted in the Republic of Belarus. Materials and methods. The case histories of 161 patients of the Republican Scientifi c and Practical Center for Pediatric Oncology, Hematology and Immunology, who underwent allo-HSCT therapy in the period from 01.11.2011 to 05.20.2021, were studied. Results. It was determined that the frequency of cGVHD with manifestations in the oral cavity among all studied cases was 11.8% and 41% of all cases of GVHD. The majority of cGVHD cases with damaged tissues of the oral cavity (81%) were registered in patients with leukemia (the proportion of patients with this diagnosis among those included in the analysis was 70%); the incidence of GVHD with oral signs in patients with ALL was 11%, with AML – the highest 23%. Conclusion. Thus, the study of archival case histories of children and young adult recipients of allo-HSCT allows us to conclude that cGVHD with manifestations in the oral cavity is an actual problem.
