Анализ клинических случаев постнатальной генерализованной ЦМВИ у доношенного и недоношенного ребенка

Abstract

Инфекции перинатального периода не теряют своей актуальности. В данной статье авторы представили проблему тяжелого течения приобретенной цитомегаловирусной инфекции (ЦМВИ), как оппортуниста у новорожденных, на примере клинических случаев. Описаны проявления инфекции на фоне несостоятельности иммунной системы и в сочетании с другими возбудителями у доношенного и недоношенного младенца. Изложена тактика ведения таких пациентов. Освещена важность ранней диагностики и необходимость лечения приобретенной ЦМВИ у новорожденных с сопутствующими состояниями, сопровождающимися иммуносупрессией.

Human cytomegalovirus is the leading cause of congenital and acquired viral infections in newborns. While acquired infection is often asymptomatic in most newborns, immunocompromised ones are at an increased risk for severe manifestations. In this review the authors presented the clinical cases of severe postnatal acquired cytomegalovirus infection as an opportunist. Clinical manifestations of the infection in full-term and premature newborns with underlying immunodeficiency caused with other infections are described. Management of such patients and diagnostic issues that can occur are outlined. The importance of early diagnosis and proper treatment of acquired CMV in infants with immunosuppression are highlighted. It’s necessary to increase awareness about postnatally acquired CMV infection in such patients because of its highly heterogeneous and nonspecific clinical presentation, which may result in an incorrect, delayed, or concealed diagnosis and treatment.
