Осложнения, возникающие при изготовлении адгезивно-шинирующего протеза

Abstract

Цель работы — исследовать осложнения, возникающие при изготовлении адгезивно-шинирующего протеза. Объекты и методы. В соответствии с целью исследования у 220 пациентов была проведена визуальная оценка состояния изготовленных адгезивно-шинирующих протезов. Результаты. Оценка адгезивно-шинирующих протезов показала, что у 70,5 % пациентов они сохранили свою целостность и не оказали негативного влияния на окружающие ткани. У 29,5 % пациентов с изготовленными адгезивно-шинирующими конструкциями были выявлены различного рода дефекты. Наибольшее число осложнений связано с нарушением краевого прилегания (30,4 %) и сколами композиционного материала (18,7 %), покрывающего армирующий компонент. Отрыв одного или нескольких шинируемых зубов, а также искусственного зуба выявили в 20,9 % наблюдений. В 24,6 % констатировали осложнения, связанные с кариозным поражением опорных и шинированных зубов. Эстетический дефект в области адгезивно-шинирующей конструкции был определен у 13,3 %. В 9,5 % у ранее шинированных зубов были выявлены клинические признаки воспаления или некроза пульпы, эндопериодонтит. Гингивит вследствие травмы слизистой оболочки маргинальной десны нависающим краем адгезивно-шинирующим протезом отмечен в 13,3 %. Заключение. Знание причин возникновения ошибок и осложнений, возникающих в процессе изготовления адгезивно-шинирующего протеза, позволяет выбрать метод лечения с оптимизированным расположением конструкции и скоординировать работу на всех этапах, что минимизирует число осложнений.

The aim of the work is to investigate the complications that arise during the manufacture of an adhesive splint prosthesis. Objects and methods. In accordance with the purpose of the study, 220 patients underwent a visual assessment of the condition of the manufactured adhesive splint prostheses. Results. The evaluation of adhesive splinting prostheses showed that in 70.5 % of patients they retained their integrity and did not adversely affect the surrounding tissues. Various defects were found in 29.5 % of patients with manufactured adhesive splints. The greatest number of complications is associated with a violation of the marginal fit of 30.4 % and chipping of the composite material of 18.7 % covering the reinforcing component. The separation of one or more splinted teeth, as well as an artificial tooth, was detected in 20.9 % of the observations. In 24.6 %, complications related to carious lesions of the supporting and splinted teeth were noted. An aesthetic defect in the area of the adhesive splinting structure was detected in 13.3 %. In 9.5 % of previously splinted teeth, clinical signs of inflammation or necrosis of the pulp and endoperiodontitis were detected. Gingivitis due to injury to the mucous membrane of the marginal gum by the overhanging edge of the adhesive splint prosthesis was noted in 13.3 %. Conclusion. Knowing the causes of errors and complications that occur during the manufacture of an adhesive splint prosthesis allows you to choose a treatment method with an optimized design arrangement and coordinate work at all stages, which minimizes the number of complications.
