Постбариатрическая гипогликемия: патогенетические механизмы, диагностика и ведение (обзор литературы)

Abstract

Растущая распространенность ожирения определяет актуальность проблем, связанных с лечением этой патологии. Несмотря на то что модификация образа жизни является ключевым аспектом в лечении ожирения, медикаментозная терапия и бариатрические вмешательства остаются наиболее эффективными методами лечения. Цель исследования – провести систематический обзор литературы по вопросу диагностики и лечения позднего осложнения бариатрических вмешательств – постбариатрических гипогликемий (ПБГ). Обзор проводился с использованием данных PubMed и Google Scholar, для поиска применялись специфические слова и словосочетания, такие как «постбариатрическая гипогликемия», «диагностика постбариатрической гипогликемии», «лечение постбариатрической гипогликемии». Для понимания последствий потенциальных вмешательств рассмотрены механизмы развития ПБГ. Изучены вопросы диагностики ПБГ с учетом особенностей патологии. Приведены рекомендации по модификации образа жизни и принципы нутритивной терапии и физической активности. Описан алгоритм выбора медикаментозной коррекции ПБГ и анализ эффективности лекарственной терапии, направленной на предотвращение и облегчение симптомов ПБГ.

The growing prevalence of obesity determines the relevance of issues associated with the treatment of this pathology. Despite the fact that lifestyle modification is a key aspect in the treatment of obesity, the use of drug therapy and bariatric interventions are the most effective treatment methods. Purpose of the study is to conduct a systematic review of the literature on the diagnosis and treatment of the most common complication of bariatric interventions: postbariatric hypoglycemia (PBH). The review was conducted using PubMed and Google Scholar data, specific words and phrases were used for the search, such as "postbariatric hypoglycemia", "diagnosis of postbariatric hypoglycemia", and "treatment of postbariatric hypoglycemia". To understand implications of potential interventions, mechanisms underlying PBH were explored. Issues related to PBH diagnosis were considered, taking into account the peculiarities of this pathology. Recommendations on lifestyle modification and principles of nutritional therapy and physical activity are given. An algorithm for choosing a drug correction of PBH and an analysis of the effectiveness of drug therapy aimed at preventing and alleviating the symptoms of PBH is presented.
