Герпесвирусная инфекция и ювенильный идиопатический артрит: есть ли взаимосвязь?

Abstract

Введение. Обсуждается роль герпесвирусной инфекции в клинике проявления ревматоидного артрита у детей. Сегодня ведутся дискуссии о том, являются ли вирусы пассивными «свидетелями» аутоиммунных нарушений или поддерживают антигенную стимуляцию клеток иммунной защиты макроорганизма. Представляет особый интерес уточнение роли данной инфекции в развитии заболевания, наличие клинических параллелей у детей. Цель. Оценить инфицированность лимфотропными вирусами у пациентов с ювенильным идиопатическим артритом (ЮИА) с позиции клинико-лабораторной характеристики и установить влияние на эффективность проведения базисной терапии заболевания. Материалы и методы. Клинико-лабораторное исследование было проведено среди 125 детей с диагнозом «ювенильный идиопатический артрит». Всем пациентам проводился иммуноферментный анализ (ИФА) для выявления антител к вирусам семейства Herpesviridae: вирусу Эпштейна – Барр (ВЭБ) и цитомегаловирусу (ЦМВ). Результаты. У пациентов с ЮИА обнаружено наличие антител к ВЭБ/ЦМВ у 50 (40,0±4,38%) человек (р=0,003). Антитела только к ВЭБ обнаружены среди 16 (32,0±6,6%), к ЦМВ – среди 34 (68,0±6,6%) человек. Наличие антител к ВЭБ/ЦМВ имели 37 (74,0%) пациентов с суставной формой и 13 (26,0%) пациентов с системной формой заболевания. Установлено достоверное влияние на ряд клинических проявлений суставного синдрома у пациентов с ЮИА в виде более агрессивного течения заболевания в условиях инфицирования ВЭБ/ЦМВ. Эффективность проводимой базисной терапии в группе пациентов с сопутствующей ВЭБ/ЦМВ-инфекцией выявлялась на 1,5 месяца позже (р=0,001). Заключение. Определение антител к ВЭБ/ЦМВ у пациентов с ЮИА, возможно, будет способствовать пониманию патогенеза развития клинических симптомов заболевания, а также более эффективному лечению.

Introduction. The role of herpesvirus infection in the clinical manifestation of rheumatoid arthritis in children is discussed. Today, the following is a debatable position: are viruses passive "witnesses" of autoimmune disorders or do they support antigen stimulation of the cells of the immune defense of the macroorganism? Of particular interest is the clarification of the role of this infection in the development of the disease, the presence of clinical parallels in children. Purpose. To assess the infection with lymphotropic viruses in patients with juvenile idiopathic arthritis from the standpoint of clinical and laboratory characteristics and to establish the impact on the effectiveness of basic therapy for the disease. Materials and methods. A clinical and laboratory study was conducted among 125 children diagnosed with juvenile idiopathic arthritis. All patients underwent enzymelinked immunosorbent assay (ELISA) to detect antibodies to viruses of the Herpesviridae family: Ebstein – Barr virus (EBV) and cytomegalovirus (CMV). Results. In patients with JIA, antibodies to EBV/CMV were detected in 50 (40.0±4.38%) people (p=0.003). Antibodies only to EBV were detected among 16 (32.0±6.6%), to CMV among 34 (68.0±6.6%). Antibodies to EBV/CMV were present in 37 (74.0%) patients with the articular form and 13 (26.0%) patients with the systemic form of the disease. A reliable effect on a number of clinical manifestations of the articular syndrome in patients with JIA, caused by a more aggressive course of the disease under infection conditions, was established. The effectiveness of the basic therapy in the group of patients with concomitant EBV/CMV infection occurred 1.5 months later (p=0.001). Conclusion. Determination of antibodies to EBV/CMV in patients with JIA may contribute to understanding the pathogenesis of the development of clinical symptoms of the disease, as well as more effective treatment.
